Requirements for Registration

Age:  

• Child must be five years old by Sept. 1st to enter Kindergarten.  
• Child must be six years old by Sept. 1st to enter first grade.

Documents needed:

• Birth Certificate
• Shot Records
• Proof of Address
• Registration Fee - $25.00
• Social Security Card
• Report Card for Grades 2 - 8

All forms listed to the right must be completed for Registration.  There are also hard copies of the forms available in the School Office.
